WCA: Workmen’s Compensation Act
Patients injured on duty will be assisted by us on condition that the following legal requirements as indicated by WCA are met upon examination:
Required Documentation
- All WCA cases need to have a 1st medical report issued by a medical doctor / casualty clinician.
- Patient ID or Drivers’ License
- Non SA citizens must have a Passport with a valid work permit.
- Referral Note from examining / referring doctor
- Copies of all documentation will be made for every case.
- In the event that an injury occurred longer than 12 hours before consulting, the patient needs to submit a statement stating the reason for the delay (delay report).
- In the event of the injury occurring as a result of a motor vehicle accident, the patient needs to submit an accident report.
Employer Report of an accident (WCL 2)
- Must be completed in full.
- Must be signed by the employer.
- The form must be legible.
Ultrasound requests
- The area requested must be related to the area indicated as injured on the WCA form.
- Must have a 1st medical report attached.
- Detailed motivation letter from the referring doctor is required if procedure date differs from date of injury as authorization is required with WCA.
- Referral note
- WCL 2 form (Employer Report).
- Patient ID.
- If documentation is incomplete, the patient must take responsibility for the account.
CT requests
- The area requested must be related to the area indicated as injured on the WCA form.
- Must have a 1st medical report attached.
- Detailed motivation letter from the referring doctor is required if procedure date differs from date of injury as authorization is required with WCA
- Referral note
- WCL 2 form (Employer Report).
- Patient ID.
- If documentation is incomplete, the patient must take responsibility for the account.
MRI requests
- The area requested must be related to the area indicated as injured on the WCA form.
- Only a single MRI spinal region is allowed and only one MRI scan is allowed. If a 2nd follow up MRI is requested, special approval from the WCA is required.
- Must have a 1st medical report attached.
- Detailed motivation letter from the referring doctor is required if procedure date differs from date of injury as authorization is required with WCA.
- Referral note.
- WCL 2 form (Employer Report).
- Patient ID.
- In the event that the injury is older than 6 months, an approved re-opening letter is required from WCA.
- If documentation is incomplete, the patient must take responsibility for the account.
Follow-up examinations – we will require all updated documentation (e.g. Progress report, Final report, etc).
