Workmen’s Compensation cases

Patients injured on duty can be assisted once the documentation required for a Workmen’s Compensation case has been supplied. Use this checklist before visiting the practice.

Required documentation

  • A first medical report issued by a medical doctor or casualty clinician.
  • Patient identity document or driver’s licence.
  • Passport and valid work permit for patients who are not South African citizens.
  • Referral note from the examining or referring doctor.
  • A delay report if the injury occurred more than 12 hours before consultation.
  • An accident report if the injury resulted from a motor-vehicle accident.
WCL 2 Employer’s Report of an AccidentThe WCL 2 form must be completed in full, signed by the employer and clearly legible. Copies of the supporting documentation will be made for each case.

Examination-specific requirements

Ultrasound

The requested area must relate to the recorded injury. If the procedure date differs from the injury date, a detailed motivation is required for authorisation.

CT examination

The requested area must relate to the recorded injury. A detailed motivation is required when the procedure and injury dates differ.

MRI examination

Only one spinal region and one MRI scan are ordinarily allowed. A date difference requires a detailed motivation, a second follow-up MRI needs special approval, and injuries older than six months require an approved reopening letter.

All three examination types require: a first medical report, referral note, WCL 2 employer report and patient ID. Incomplete documentation may make the patient responsible for the account.
